Video Gaming License Application Submission

To submit your video gaming license application, please complete the fillable application form and forward it tojsanchez@berkeley.il.us.

All invoices will be sent directly to the addresses provided on your application to complete the payment process and ensure your license is issued in a timely manner.

Please note that all application deadlines are December 29, 2025, to ensure your new license is issued and ensure business is in compliance by January 1, 2026.
